All patients are assigned a registered nurse case manager who coordinates with all the other Home Health disciplines and the patient’s health care providers to develop the specific patient’s plan of care. Skilled Nursing services that WVP Home Health provides are:

Physical Therapy Services
Our contracted physical therapist assesses the patient’s musculoskeletal system and educates patients/families on safety and mobility. Physical Therapy services that WVP Home Health provides are:

Occupational Therapy Services
Our contracted occupational therapist evaluates the patient’s ability to perform activities of daily living, determines barriers to independence and develops programs that help restore function. The evaluation includes identifying limitations in muscle strength, coordination, range of motion and problem solving skills. * Lymphedema treatment is provided by occupational therapists that are certified in Lymphedema treatment and are employees of WVP Home Health.

Speech Therapy Services

A Speech-Language Pathologist (SLP) specializes in the evaluation and treatment of swallowing, cognition/thinking skills, and communication disorders.

SLP services aim to improve a person’s quality of life through regaining the ability to eat and swallow, improving cognitive skills for increased independence, and helping to regain communication skills to express wants, needs, and ideas. The SLP also works with caregivers to help maintain the skills for long-term success.

Provider Coordination
WVP Home Health works collaboratively with all of the patient’s health care providers. Our team works diligently to inform, dialog and problem solve the patient’s health care needs with the patient and the appropriate members of the patient’s direct health care team to ensure the best possible health outcome
